چکیده

Introduction Non-invasive ventilation (NIV) is the first line supportive treatment in case of acute hypercapnic respiratory failure (AHRF). NIV efficacy is continuously monitored using clinical parameters (respiratory frequency, use of accessory respiratory muscles). The assessment of NIV efficacy however usually requires repeated blood gas analysis after 30 or 60 minutes of treatment. A reliable non-invasive technique to continuously monitor PaC02 during NIV could simplify this evaluation and allow an earlier adaptation of ventilator settings.

Active cycle of breathing techniques in non-invasive ventilation for acute hypercapnic respiratory failure.

We hypothesised that applying the active cycle of breathing techniques (ACBT) in patients with acute hypercapnic respiratory failure undergoing non-invasive ventilation would improve patient outcome. Thirty-four patients were randomised so that 17 patients with acute hypercapnic respiratory failure received the ACBT and non-invasive ventilation (ACBT group), and 17 patients received non-invasiv...

[Non-invasive ventilation in chronic obstructive pulmonary disease patients with acute respiratory hypercapnic failure in a conventional hospital ward].

OBJECTIVE To analyze chronic obstructive lung disease (COPD) subjects in acute hypercapnic failure who were treated with non-invasive mechanical ventilation in a general respiratory ward. METHODS This was a two-year prospective study of 35 patients with acute exacerbation of COPD and mean FEV1/FVC relation in stable condition of 55.3 +/- 14.8% of predicted that were treated with positive pres...

Early Effectiveness of Noninvasive Positive Pressure Ventilation on Right Ventricular Function in Chronic Obstructive Pulmonary Disease Subjects with Acute Hypercapnic Respiratory Failure

Introduction: Noninvasive positive pressure ventilation (NIPPV) has become an integral tool in the management of acute hypercapnic respiratory failure (AHRF) in chronic obstructive pulmonary disease (COPD). This study was performed to evaluate the early effects of NIPPV on pulmonary artery pressure (PAP), serum N-terminal pro BNP (NT-proBNP), and ventilatory parameters in the COPD patients with...

Comparison of Transcutaneous and Capillary Measurement of PCO2 in Hypercapnic Subjects.

BACKGROUND Measurement of PCO2 is vital in determining effective alveolar ventilation. However, obtaining capillary PCO2 by a skin prick of the earlobe is painful, and nocturnal measurements disturb sleep. End-expiratory measurement of PCO2 is also well established, but there is a low precision in predicting arterial or capillary CO2. The purpose of the study was to evaluate nocturnal measureme...

End-tidal carbon dioxide monitoring using a naso-buccal sensor is not appropriate to monitor capnia during non-invasive ventilation

BACKGROUND In acute respiratory failure, arterial blood gas analysis (ABG) is used to diagnose hypercapnia. Once non-invasive ventilation (NIV) is initiated, ABG should at least be repeated within 1 h to assess PaCO2 response to treatment in order to help detect NIV failure. The main aim of this study was to assess whether measuring end-tidal CO2 (EtCO2) with a dedicated naso-buccal sensor duri...
